There are important changes to your benefits for 2012 that may require you to take action during the Benefit Enrollment and Change Period that begins on October 3, 2011 and ends on October 28, 2011 for all participating plans.

All benefits, except the Employee Reimbursement Account (ERA) program, automatically continue from year to year. You must re-enroll in the ERA program every year.

This is typically the only time during the year when you are eligible to make benefit plan changes unless you have a life event (marriage, birth, divorce, etc.) that allows you to add or change benefits. Even If you do not want to make any changes to your benefits for 2012, please take a few moments to review the fall enrollment materials to ensure that your current benefits and health plan will be available to you next year. If you do not want to make any changes, you do not have to submit any applications.

You may take the following actions during the upcoming benefit and enrollment change period:

All benefit enrollments and changes made during this period will be effective January 1, 2012.

All insurance premiums are available on the UW Service Center website. Both 2011 and 2012 premiums are listed on the website.

Changes that Apply to Multiple Benefit Plans

State Group Health Insurance | It's Your Choice Period: October 3–28

Eligibility: All active, benefit-eligible UW System employees may enroll in coverage during the open enrollment period.

New for 2012

See the State Group Health Insurance It's Your Choice page for plan booklets, premium information, and more details about changes for 2012. You should also read this information to confirm that your current health plan will be available in 2012 and to review any changes to your coverage. Your current health plan will send you a letter in early October to alert you about changes for 2012, including a list of providers that will no longer be available.

Dental Wisconsin Dental Insurance | Open Enrollment and Change Period: October 3–28

Eligibility: All active, benefit-eligible UW System employees may enroll in coverage during the open enrollment period. Current subscribers may also change from the Preferred Provider Organization (PPO) to the Select Plan or from the Select Plan to the PPO.

See the Dental Wisconsin page for enrollment, plan, and premium information.

EPIC Benefits+ | Special Enrollment Period: October 3–28

Eligibility: All active, benefit-eligible UW System employees may enroll in coverage during this special enrollment period. Current subscribers may also add or remove the optional vision insurance benefit.

New for 2012:  The excess medical portion of the plan was removed and a Hospital and Surgery Benefit was added. Under the new benefit, you will receive $100 for each day of hospitalization in excess of three days or following an outpatient surgery performed at a hospital or ambulatory surgical center.

See the EPIC Benefits+ page for enrollment, plan, and premium information.

Anthem DentalBlue Dental Insurance | Change Period: October 3–28

Eligibility: Employees who have Anthem DentalBlue coverage in force as of October 1, 2011. Plan will not be open to new enrollees for 2012. Plan will no longer be available in 2013.

Eligibility: All active, benefit-eligible UW employees, except those in the following positions: LTE, Research Assistant, Fellow, Scholar, Graduate Intern/Trainee, or Post-Doctoral Fellow/Trainee.

New for 2012: If you have coverage under State Group Health Insurance, you will have increased out-of-pocket costs due to new coinsurance provisions. The coinsurance that you pay for eligible services qualifies for reimbursement under the ERA program.

This is the first year that the ERA enrollment period does not extend beyond the It's Your Choice health insurance enrollment period. The enrollment period for all participating plans ends on October 28th.

See the ERA page for details and enrollment information.

UW Employees, Inc. Life Insurance | Open Enrollment Period: October 3–28

Eligibility: All active, benefit-eligible UW System employees may enroll in coverage during the enrollment period.

New for 2012: Coverage levels will slightly increase but there will be no premium increase. Effective October 1, 2011, Minnesota Life will become the new carrier of the plan (Country Life Insurance Company was the previous carrier). Due to the change in carrier, beneficiary designations currently on file will no longer be valid as of January 1, 2012.

See the UW Employees, Inc. Life Insurance page for enrollment, plan, beneficiary, and premium information.

Individual and Family Life Insurance | Annual Increase Option (AIO) Period: October 3–28

See the Individual and Family Life Insurance page for coverage increase, plan and premium information.
